As the debate blazes, deep-pocketed big tobacco investors are buying up e-cig companies, injecting millions of dollars into the market and banking on a bright future for the devices. More than 100 e-cigarette companies are now jockeying for the business of smokers and nonsmokers alike.

One chemical, known as 6-methyl nicotine, has been shown in rodent experiments to be far more potent than nicotine in targeting the brain’s nicotine receptors and more toxic than nicotine. Smoking harms nearly every organ in the body ― heart and blood vessel disease; lung disease; cancer (almost anywhere in your body); and impacts to your bones, eyes, teeth, gums, fertility and pregnancy. Individual businesses can prohibit the use of vape devices indoors but this is not required by state law. More adults 18 to 24 years old used e-cigarettes (11.0%, or roughly 3.4 million people) compared with adults 25 to 44 (6.5%) or 45 and older (2.0%). The latest research, published in a July National Center for Health Statistics data brief, shows that in 2021, e-cigarette use was highest among adults 18 to 24 years old.

Other compounds that have been detected in aerosols include acetamide, a potential human carcinogen [5], and some aldehydes [69], although their levels were minimal. Interestingly, the existence of harmful concentrations of diethylene glycol, a known cytotoxic agent, in e-liquid aerosols is contentious with some studies detecting its presence [4, 68, 70,71,72], and others finding low subtoxic concentrations [73, 74]. In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Figure 1 lists the main compounds detected in aerosols derived from humectant heating and their potential damaging effects.

The program’s curriculum includes a four-part series of lessons, with one set for fifth through eighth graders and another for high schoolers. Nicotine yields for different e-cigarette brands have been reported from 2 to 313 µg puff−1 while for conventional cigarettes smoke the values ranged from 170 to 232 µg puff−16,31,36,48,64. Nicotine poisoning refers to the toxic effects of consuming nicotine, a chemical in all tobacco products. A recent increase in poisonings is due to liquid nicotine, a product in the popular e-cigarette. Symptoms include vomiting, rapid heart rate, unsteadiness and increased salivation. The comprehensive review of the evidence finds that almost all of the 2.6 million adults using e-cigarettes in Great Britain are current or ex-smokers, most of whom are using the devices to help them quit smoking or to prevent them going back to cigarettes. It also provides reassurance that very few adults and young people who have never smoked are becoming regular e-cigarette users (less than 1% in each group).

Indeed, one of the limitations of the study was that human cells are not exposed to e-liquids per se, but rather to the aerosols where the concentrations are lower [86]. In this line, the maximum concentration tested (1000 µM) would correspond to approximately 80 to 150 ppm, which is far higher than the levels found in aerosols of some of these compounds [84]. Moreover, on a day-to-day basis, lungs of e-cigarette users are not constantly exposed to these chemicals for 24 h at these concentrations. Similar limitations were found when five of seven flavourings were found to cause cytotoxicity in human bronchial epithelial cells [87]. Median odds ratio (MOR), which shows the difference between two respondents with identical student-level characteristics from two randomly chosen different schools, was calculated to express between-school variance on the odds ratio scale [36]. FDA regulates the manufacture, import, packaging, labeling, advertising, promotion, sale, and distribution of ENDS, including components and parts of ENDS but excluding accessories. An outbreak of EVALI in late 2019 and early 2020 put thousands of people in the hospital. Since then, EVALI cases have been declining, but people who vape can still get EVALI. Research from the CDC shows that vaping among youth has declined somewhat since 2020.

In regard to COVID-19 pandemic, the actual literature suggests that nicotine vaping may display adverse outcomes. Therefore, follow up studies are necessary to clarify the impact of e-cigarette consumption on human health in SARS-CoV-2 infection. Recently, a commonly commercialized crème brûlée-flavoured aerosol was found to contain high concentrations of benzoic acid (86.9 μg/puff), a well-established respiratory irritant [88]. When human lung epithelial cells (BEAS-2B and H292) were exposed to this aerosol for 1 h, a marked cytotoxicity was observed in BEAS-2B but not in H292 cells, 24 h later.
Until we know more, it is probably best to avoid these products whenever possible, including secondhand smoke. However, vaping early on may increase the chances of smoking ordinary cigarettes later in life. Vaping has become an epidemic among young people in the United States.
Prior to that, it was the Wild West, where a variety of companies started making and selling products with their own personal spin. Per the survey, 10.0% of students (2.80 million) reported current use of any tobacco product; 12.6% (1.97 million) high school students and 6.6% (800,000) middle school students reported current use of any tobacco product. Importantly, the researchers found strong evidence of the program's effectiveness among key subgroups defined by race, ethnicity, gender, higher levels of nicotine dependence, smoking and other substance use, mental health challenges, and household dysfunction. "For many years, healthcare providers, teachers, and parents have been asking how to help teens quit vaping," said Dr. Amanda Graham, Chief Health Officer at Truth Initiative and principal investigator of the study.
Recent guidance from the Center for Tobacco Products indicates the importance of switching completely away from combustible cigarettes for those who are also using e-cigarettes," she added. The investigators revealed that daily vs nondaily use of e-cigarettes was linked to higher overall rates of quitting combustible cigarettes (12.8% vs 6.1%). The adults who used e-cigarettes in 2019 were more likely to stop smoking traditional cigarettes compared with those who used the e-cigarettes between 2014 and 2015 (12.0% vs 5.3%). There was limited evidence that flavor or device type impacted cigarette cessation. The findings suggest that daily e-cigarette use may help some patients to quit smoking combustible cigarettes.
But e-cigarettes haven't proved to be safer or more effective than nicotine-replacement medications in helping people stop smoking. He was a founding member of the Society for Nicotine and Tobacco Research.
