The study showed that the e-liquids of certain cig-a-like brands contain high levels of nickel and chromium, which may come from the nichrome heating coils of the vaporizing device. Cig-a-likes may also contain low levels of cadmium, a toxic metal also found in cigarette smoke that can cause breathing problems and disease.11 More research is needed on the health consequences of repeated exposure to these chemicals.
Vapor products have many names, including electronic nicotine delivery systems (ENDS), electronic cigarettes (e-cigarettes), JUULs or vape pens. All adults who smoke conventional cigarettes or other combustible (burned) tobacco products should be advised to quit smoking at the earliest opportunity, recognizing that quitting is hard and often takes repeated, dedicated efforts. Individuals can also seek cessation support by calling QUIT-NOW or ACS-2345.
In response to this issue, the PACT Act Amendment prohibits sellers from using the U.S. Post Office to ship e-cigarettes, vapes, flavored and smokeless tobacco products. The PACT Act applies to all shipments in interstate commerce (15 U.S.C. § 376) as well as to all delivery sales. Some people who smoke choose to use both cigarettes and e-cigarettes at the same time on an ongoing basis, whether they are trying to quit or not.

The FDA recently approved its first menthol-flavored electronic cigarettes for adult smokers. In contrast to the clear evidence that flavored products fueled the youth e-cigarette epidemic, every major U.S. public health authority – including the U.S. Preventive Services Task Force, the CDC and even the FDA itself – has found there is inadequate evidence to conclude that e-cigarettes are effective at helping smokers quit. E-cigarettes are battery-operated devices used for a type of smoking called vaping.
The length of time spent vaping can be much longer than smoking a standard cigarette. While most cigarettes are smoked within two to five minutes, e-cigarettes can last up to 20 minutes, delivering more nicotine and damaging chemicals to the lungs. In addition, some vaping mixtures can contain 20 times the nicotine that a single cigarette contains. These substances can be harmful to the body, with different flavourings helping to mask these poisons when they are taken into the body. A 2018 report from the National Academies Press (NAP) found significant evidence that taking a puff from a nicotine e-cigarette triggers an increase in heart rate. In summary, it seems that either smoking or nicotine vaping may adversely impact on COVID-19 outcome. However, additional follow up studies are required in COVID-19 pandemic to clarify the effect of e-cigarette use on lung and cardiovascular complications derived from SARS-CoV-2 infection. It is also noteworthy that among the 3 most cytotoxic vapours for HUVEC evaluated in the Putzhammer et al. study, 2 were nicotine-free, which suggests that nicotine is not the only hazardous component in e-cigarettes [24]. However, while vape products can reduce the amount of tar and other chemicals a person inhales, they can increase a person’s nicotine dependency. The National Institute on Drug Abuse (NIDA) states that around half of all smokers try to quit every year, while only 6% manage to do so.
The FDA has the legal authority to regulate tobacco products containing nicotine from any source, including synthetic nicotine. In 2016, the FDA established a rule for e-cigarettes and their liquid solutions. The Lung Association remains extremely troubled about the rapid increase of youth using these products and has repeatedly called upon the Food and Drug Administration (FDA) to increase their oversight and scrutiny of these products to protect kids. There are different types of e-cigarette, including vape pens, vape bars, pod devices, mods, and cigalikes. Strong decisive action is needed to prevent the uptake of e-cigarettes based on the growing body of evidence of use by children and adolescents and health harms. How a country approaches ENDS will depend on factors particular to its situation. In others they are regulated as consumer products, as pharmaceutical products, as tobacco products, in other categories or totally unregulated. Nicotine exposure in pregnant women can adversely affect the development of the fetus. Further, the consumption of nicotine in children and adolescents has negative impacts on brain development, leading to long-term consequences for brain development and potentially leading to learning and anxiety disorders.

The success of all these enterprises hinges on the claim that e-cigarettes are healthier than traditional cigarettes. Companies like to paint a black-and-white picture of a new era of safe smoking. "Cigarettes, you've met your match," NJOY proudly proclaims in its Super Bowl ads. E-cigarettes and vaping are part of a trend going back at least nine years in the U.S. First publicized as a safer alternative to smoking tobacco, vaping caught on because it didn’t contain the carcinogens or tars found in most smoking tobacco products. Also, vaping was supposed to eliminate the dangers of secondhand smoke to those nearby.
"This is the first study to demonstrate the effectiveness of any quit vaping program for adolescents. It’s also a program that has been used by more than 780,000 young people nationwide since Truth Initiative launched it in 2019," Dr. Amanda Graham, chief health officer at Truth Initiative and principal investigator of the study, said in an email. Her team tracked rates of both vaping and smoking among nearly 5,000 Columbus-area people diagnosed with lung cancer, and compared them to a group of almost 27,300 people without cancer. It found that there was not enough evidence to confirm its products did not harm public health. The FDA has banned the sale of flavored nicotine products in the US unless a company has received their expressed approval.
Based on the current literature, it seems that several factors have led to the success of e-cigarette use as a smoking cessation tool. First, some e-cigarette flavours positively affect smoking cessation outcomes among smokers [102]. Second, e-cigarettes have been described to improve smoking cessation rate only among highly-dependent smokers and not among conventional smokers, suggesting that the individual degree of nicotine dependence plays an important role in this process [97]. Third, the general belief of their relative harmfulness to consumers' health compared with conventional combustible tobacco [103]. And finally, the exposure to point-of-sale marketing of e-cigarette has also been identified to affect the smoking cessation success [96].
The Centers for Disease Control and Prevention (CDC) report that vaping nicotine can permanently affect brain development in people under the age of 25. The researchers concluded that vaping with and without nicotine disrupts typical lung function in otherwise healthy people. The researchers reported a number of adverse effects on both types of cells, including toxicity, oxidation, and inflammation. However, these results aren’t necessarily generalizable to vaping in real life. A 2019 study assessed data from a nationwide survey of nearly 450,000 participants and found no significant association between e-cigarette use and heart disease. The authors also described moderate evidence suggesting that taking a puff from an e-cigarette increases blood pressure.
According to the CDC, 1 in 4 deaths in the U.S. result from cardiovascular disease caused by cigarette smoking. Teens who smoke can end up with lungs that are smaller and weaker than the lungs of teens who don’t smoke. Quitting smoking is one of the best things you can do for your health — smoking harms nearly every organ in your body, including your heart. Perhaps contrary to what many would expect, vaping marijuana may have as many negative health effects as nicotine-based vaping. This may have more to do with how vape liquids are processed than the THC itself. The tobacco industry profits from destroying health and is using these newer products to get a seat at the policy-making table with governments to lobby against health policies. The tobacco industry funds and promotes false evidence to argue that these products reduce harm, while at the same time heavily promoting these products to children and non-smokers and continuing to sell billions of cigarettes.
"It’s young people doing something terrible for them that’s supposed to be healthy," he said. There’s a whole genre of throwing-away-my-Juul videos on social media, with people tossing their vape into a river or a snowbank as dramatic music plays. New research led by Roswell Park experts in health behavior and published today in the journal JAMA Network Open suggests that daily e-cigarette use may help some people to quit using combustible cigarettes.

In this particular aspect, again the composition of the e-liquid varies significantly among different commercial brands [4, 35]. The most common and major components of e-liquids are PG or 1,2-propanediol, and glycerol or glycerine (propane-1,2,3-triol). Both types of compounds are used as humectants to prevent the e-liquid from drying out [2, 53] and are classified by the Food and Drug Administration (FDA) as "Generally Recognised as Safe" [54].
This means that e-cigarettes cannot be used in areas where smoking is banned. Patients affected by the disease have symptoms ranging from cough, chest pain, and shortness of breath to fatigue, vomiting, diarrhea, weight loss, and fever. According to the manufacturer, a single JUUL pod contains as much nicotine as a pack of 20 regular cigarettes. Although JUUL is currently the top-selling e-cigarette in the U.S., other e-cigarettes are becoming available that look like USB flash drives. Because of their shape, school teachers, administrators, and parents may not notice students using these devices in school, including in classrooms and bathrooms. Adults who use tobacco and are trying to quit should use proven quit tools, like group or phone counseling, paired with medications or nicotine replacement therapy patches, gum or lozenges. The agency says the sweet flavored e-liquids pose a "serious, well-documented risk" of enticing more young people to pick up a nicotine habit. In 2020, nearly 20% of high school students and almost 5% of middle-school students used e-cigarettes, and almost all of those kids used flavored products, the agency said in court documents.
E-cigarettes may be disposable, be refillable with e-liquid, or have a prefilled cartridge or pod for reuse. Some e-cigarettes can also be used to deliver cannabis and other drugs. Additionally, high quality epidemiology studies consistently demonstrate that e-cigarettes use increases conventional cigarette uptake, particularly among non-smoking youth, by nearly 3 times. Evidence reveals that these products are harmful to health and are not safe. However, it is too early to provide a clear answer on the long-term impact of using them or being exposed to them.
There have also been cases of e-cigarettes exploding or catching fire. Some chronic users have developed obstructions in their lung pathways that cause them to suffer 'asthma-like' symptoms. Similar to Wyoming, the state's vast rural areas might contribute to difficulty enforcing regulations surrounding youth vaping. West Virginia came in second place, with 28 percent of teens vaping last year. The large rural area may make it more difficult to enforce the state's vaping laws. Rural areas also tend to have less access to clinics and resources about the health harms surrounding vaping.
This data highlights a small part of a larger issue, despite the ban on e-cigarettes and vapes in India under the Prohibition of Electronic Cigarettes (Production, Manufacture, Import, Export, Transport, Sale, Distribution, Storage, and Advertisement) Act, 2019. The Annual Review of Physiology article summarizes what is known—and highlights where gaps in knowledge exist—about e-cigarettes and harm to the lungs. The review discusses the range of effects found in rigorous studies, including shorter-term acute effects and longer-lasting chronic impacts. As substantiated in the review, a convincing body of evidence exists that confirms e-cigarettes cause lung inflammation and injury, as well as negative health effects in multiple organs. The Policy Scan Project, by the Institute for Global Tobacco Control (at Johns Hopkins University) tracks and reports regulatory approaches to e-cigarettes around the world. In 2012 and 2013 there was a flurry of tobacco company investment in e-cigarettes, both in the United Kingdom (UK) and in the United States (US), a trend which continued in subsequent years. In 2018, 8% of Pierce County 12th graders said they had smoked a cigarette in the past 30 days. The survey also found that Minnesota youth are ill-informed about the health risks of e-cigarettes with 76.1% of 11th graders saying there is either no, slight, or a moderate risk to using e-cigarettes. 11th grade students responding to the student survey said that the top ways they got e-cigarettes included getting products from friends (72.3%), getting products at a vape shop (14.0%), and on the Internet (9.6%). Among 8th grade students, e-cigarette use nearly doubled from 2016 to 2019, and one in four 11th graders now use e-cigarettes. Students in all grades surveyed now use e-cigarettes and vapes at five times the rate of conventional cigarettes. In Australia, during the survey period, it was not legal to obtain e‑cigarettes containing nicotine without a prescription (TGA 2020).
It can often lead to debilitating symptoms and frequent hospitalizations as people age. Electronic nicotine products, which include e-cigarettes, vape pens, hookah pens, personal vaporizers and mods, e-cigars, e-pipes and e-hookahs, deliver nicotine in aerosol form without combustion. A cigarette is a narrow cylinder containing a combustible material, typically tobacco, that is rolled into thin paper for smoking. The cigarette is ignited at one end, causing it to smolder; the resulting smoke is orally inhaled via the opposite end. The term cigarette, as commonly used, refers to a tobacco cigarette, but the word is sometimes used to refer to other substances, such as a cannabis cigarette or a herbal cigarette. A cigarette is distinguished from a cigar by its usually smaller size, use of processed leaf, different smoking method, and paper wrapping, which is typically white. Not everyone is aware that there is nicotine in most e-cigarettes in varying amounts. Some pods of e-cigarettes contain as much nicotine as one pack of cigarettes (Truth Initiative, 2019).
"From a public health perspective, we have always been concerned about dual-use of both traditional and e-cig products," said study lead author Marisa Bittoni, an oncology researcher at Ohio State University in Columbus. The U.S. Food and Drug Administration and the European Union are grappling with these issues as they decide how to regulate the products. Unfettered access could leave people vulnerable to unknown health hazards, but there is also the chance that greater restrictions might hurt folks who are trying to forgo conventional—and more dangerous—tobacco products.
In addition to nicotine, e-cigarettes can also be used to inhale other drugs, such as cannabis. Retailers are not permitted to sell e-cigarettes in retail outlets to anyone. Instances of non-compliance should be reported to the local council where the premises is located. More than 5 million middle and high school students used e-cigarettes, according to the 2019 National Youth Tobacco Study (NYTS), up from more than 3.6 million the previous year. Surgeon General’s office began the work of awareness when the nation’s doctor, Jerome Adams, MD, issued a warning that vaping among youth has reached epidemic levels. Indeed, one of the limitations of the study was that human cells are not exposed to e-liquids per se, but rather to the aerosols where the concentrations are lower [86]. In this line, the maximum concentration tested (1000 µM) would correspond to approximately 80 to 150 ppm, which is far higher than the levels found in aerosols of some of these compounds [84]. Moreover, on a day-to-day basis, lungs of e-cigarette users are not constantly exposed to these chemicals for 24 h at these concentrations. Similar limitations were found when five of seven flavourings were found to cause cytotoxicity in human bronchial epithelial cells [87]. It is well known that nicotine is extremely addictive and has a multitude of harmful effects. Nicotine has significant biologic activity and adversely affects several physiological systems including the cardiovascular, respiratory, immunological and reproductive systems, and can also compromise lung and kidney function [41].
The increasing number of adolescent users and reported deaths in the United States prompted the government to ban the sale of flavoured e-cigarettes in 2020. The difference in opinion worldwide may be due to different restrictions imposed. For example, while no more than 20 ng/mL of nicotine is allowed in the EU, e-liquids with 59 mg/dL are currently available in the United States. Nevertheless, despite the national restrictions, users can easily access foreign or even counterfeit products online. "These products appear to be designed to circumvent the laws and regulations in place to protect people—especially children—from the harmful effects of smoking and tobacco use," Jordt said. The CDC advises people who have asthma to avoid tobacco smoke, yet Minnesota teens who have asthma are more likely to breathe someone else’s tobacco smoke or e-cigarette aerosol than teens who do not have asthma. Minnesota’s teens who have asthma are less likely to be protected by rules against smoking in the family’s homes or vehicles. Individuals included from the PATH study were 21 years and older daily cigarette smokers and reported any use of e-cigarettes within the previous 30 days. The new study findings follow on the heels of a report from the Substance Abuse and Mental Health Services Administration (SAMHSA) showing growing numbers of e-cigarette users.

In some instances, because of its appearance and the vapours it emits, an e-cigarette may be confused with a regular cigarette. Other types of e-cigarettes, however, are similar to a cigar, a smoking pipe, or a ballpoint pen in appearance.
